A long-term-care gap is the difference between the full cost of suitable care and the benefits, income, and assets available to pay for it. Germany's Pflegeversicherung (social long-term-care insurance) pays defined benefits by Pflegegrad (care grade) and setting. It is partial cover, so accommodation, meals, investment costs, and uncovered care can remain.

Why it matters

Health insurance and long-term-care insurance solve different problems. Pflegeversicherung does not promise to reimburse every invoice. The household gap also changes with home care, day care, residential care, unpaid family care, region, and how needs develop.

Calculate the gap without false precision

Use one monthly scenario at a time:

care gap = total care-related cash need - applicable benefits - care recipient's available income

Suppose a residential-care scenario costs €4,200 per month in total. Assume the applicable insurance and other care benefits cover €1,800 and the recipient can direct €1,500 of monthly income to the bill. The illustrative gap is €4,200 - €1,800 - €1,500 = €900 per month. This is scenario arithmetic, not a German tariff: actual 2026 benefits depend on Pflegegrad, setting, benefit combination, and eligibility [1][2].

An unpaid relative's time is not financially free. Reduced working hours can create lost income, pension effects, and exhaustion. German social insurance may credit qualifying carers with pension contributions, but the conditions must be checked [3].

A 2026 review rule

Use the care fund's current benefit notice and a current provider quotation. Do not extrapolate one national average to a specific household. Professional benefits counselling or legal advice is proportionate where assets, maintenance claims, cross-border residence, or Sozialhilfe (social assistance) may matter.
